Services in the home and coordination of care to assist seniors and individuals with disabilities to remain healthy in their home and community as long as possible.
Long-term Care Medicaid Waiver Programs
These long-term care programs are Medicaid funded and off assistance in home to help make it possible for people to stay in their homes as long as possible and delay institutional/nursing home placement. The services available through these programs may include case management, homemaker services, personal care, and respite care. Individuals applying for assistance through a Medicaid program must meet certain medical and financial requirements.
The long-term Medicaid Waiver programs available through ATRC include:
Alabama Community Transition (ACT) Waiver: The ACT waiver assists clients who wish to transition from a nursing facility back out into the community.
Elderly and Disabled (E&D) Waiver: Counseling and advocacy with Medicare and Medicaid, prescription drug coverage, and long term care coverage.
Technology Assisted (TA) Waiver: The TA waiver provides services to adults with complex skilled medical conditions who would otherwise require the care of a skilled nursing facility.
Non-Medicaid Based Support Programs
Alabama Cares: The Alabama CARES program provides temporary support to family caregivers.
