Home and Community based services are provided in the home, with coordination of care, to assist seniors and individuals with disabilities to remain healthy in their home and community as long as possible. These services are made possible through Medicaid Waiver programs.

Long-term Care Medicaid Waiver Programs

These long-term care programs are Medicaid funded and offer assistance in home to help make it possible for people to stay in their homes as long as possible and delay institutional/nursing home placement. The services available through these programs may include case management, homemaker services, personal care, and respite care. Individuals applying for assistance through a Medicaid program must meet certain medical and financial requirements:

  • Individuals must be unable to perform many activities of daily living independently and be at risk for nursing home placement.
  • Individuals must have full Medicaid benefits.
  • Individuals who have incomes 300% of the poverty level or below and limited resources may qualify for full Medicaid through the deeming process.

The long-term Medicaid Waiver programs available through ATRC include:

Alabama Community Transition (ACT) Waiver: The ACT waiver assists clients who wish to transition from a nursing facility back out into the community. To learn more about this Medicaid Waiver program click the button below.

Personal Choices

Personal Choices, a consumer-directed program, is available to individuals enrolled in the Medicaid Waiver Program. The program allows clients to self-direct their care, offering them more choice, flexibility, and control in managing their daily lives. Clients decide what help is needed, who they want to provide the assistance, and when the services will be provided. All Personal Choices clients have the following supports in place to help them be successful in the program:

Counselor – who advises the client through enrollment and orients them of their responsibilities, develops personal support plan and budget, provides monthly monitoring, and provides resources to assist client as needed.
Financial Management Services Agency – serves as the clients’ bank and accountant for the program. Keeps track of budget and expenses, pays clients’ employees, and files taxes for client and employee.
Representative – clients may allow someone to act as their representative if they do not feel comfortable managing their care alone.
