The Alabama Community Transition (ACT) Waiver assists clients who wish to transition from a nursing facility back out into the community. Clients are assisted with locating and acquiring housing and provided with home maker services, personal care, home delivered meals, caregiver respite (skilled or unskilled), skilled nursing and other services. Clients on this program may also receive assistance with medical equipment and supplies and the installation and of a personal emergency response system (PERS).

Eligible clients must meet medical and financial guidelines.
