The Medicaid Elderly and Disabled (E&D) Waiver program provides services for seniors and the disabled whose needs would otherwise would require Nursing Home placement. A case manager works with clients and caregiver to establish a custom plan of care. This plan of care may be comprised of  services including homemaker, personal care, respite, adult day health, and home delivered meals. The goal of this program is to maintain clients in their own homes surrounded by their family and friends for as long as is possible.

Eligible clients must meet medical and financial guidelines.
